Marine ich - best treatment?

OK here's what I have.
90 gallon FO
1 - 5" V-tail grouper
1 - 5" yellow tang
1 - 5" dogface puffer

all eating normally, one little spot on the tangs tail fin.

What's the best course of action? Meds? Water changes? Hyposalinity?
 
Personally, I wouldn't jump to interventions for one spot, which may or not be c. irritans. Watch for it to disappear and see if the symptoms return but more pronounced (more spots, spread to other fish). If that's the case and ich is confirmed, performing hyposalinity in the main tank would be easiest (no live rock, correct?)

That's correct, no invertabrates other than one lonely snail. I figure I'll just do a normal small water change and keep a real close eye on things.

Hypo works well if your diligent about maintaining the salinity at therapeutic levels. A well calibrated refract is necessary. Three weeks after all symptoms disappear is adequate. When you raise salinity back up, do it over the course of 5-7 days.
 
If symptoms don't appear to get better do a FW dip. If the spots fall off then it is velvet and hypo won't work. If the white dots stay on it is ich.

Well, good news and bad news. All this started about two weeks before we went off to San Francisco for a week.
The grouper was acting funny before we left, being reclusive and not eating much. Our house sitter did a good job of taking care of things, but the grouper passed away while were out of town. Not sure exactly what happened to him.
But, not a sign of ich since we have been back. The yellow tang and the puffer are both very active and eating well. Wednesday will mark four weeks, so a couple more weeks and I will slowly bring the salinity back up.

ICH UPDATE - 5 WEEKS IN

Well last weekend, my little white friends are back on the tang and puffer. I have held the salinity at 1.0085-1.009 for about 5 weeks now.

Should I just do a water change (at 1.009) and keep it there for another month or so? Or should I try something else? Both fish are eating / acting normally.
