This might be a secondary bacterial infection, which is not uncommon in fish that have had ich. IMO/IME, the garlic alone is not going to fully take care of the problem. Do you have a quarantine tank? How many fish have been exposed and how large are the fish? What size tank? You'll need to treat the parasite and then the secondary problems, otherwise you'll keep ending up where you are now.

There are only two easily-accessible treatments available that successfully end the ich life-cycle. The first is hyposalinity and the second is copper.
Porcupine puffers do not handle copper treatments very well. This is because copper treatment causes fish to produce excess mucus lining on their body. Because porcupine puffers already have a higher-than-average amount of body mucus, copper treatment can cause respiratory problems for them -- so hyposalinity as absolutely the way to go.
You will basically want to put all of your fish in a quarantine tank for 4-6 weeks under hyposaline conditions (SG at 1.009). There is a lot of info on exactly how to go about this in the fish disease/treatment board on this site. It's a pretty straightforward process and will have your fish happy and healthy in no time.
To aid in fighting secondary bacterial infections, make sure to keep water quality very clean, feed a varied diet and supplement with beta glucan.
