your last fish that died?

Sore subject, but heres mine...

My 2.5" lemonpeel kicked the bucket today. I had let it get harassed too much by my passer angel, so it just stayed in a "cave" and didnt eat. By the time I caught it to put it into the acclimation box, it was too late.

The way it was eating in QT, I thought it was going to be bulletproof.

anyone else?
 
my sunburst fathead anthia came in from shipping and was never right. I put it in QT but couldn't keep itself up right and just laid on its side. It died about 2 days later.
 
About three months ago the eventual male true percula (was not sexually mature) died. The eventual female ate everything and scared the smaller clown away from the food. She then took to beating on him when I was not around. Fins were torn in the morning, by the afternoon he was dead.
 
Came home from work a few months ago to find the six-spot sleeper goby dead on the powerhead. Had him for a year and he was a fat hog. No idea what killed him.
 
My carpenter wrasse.... I decided not to pay attention to a friend and added a mystery wrasse. I only saw the mystery chase the carpenters once or twice but I came home to it 10' from my tank. Oh and I do have a hood.
 
other day lubbocks fairy managed to somehow jump through my screens on the top of the tank and was dried up when I got home.
 
My most recent loss, lost my longnose hawkfish about two weeks ago. I really liked that fish..... he was among only two fish that survived the used tank purchase and move.

I am guessing that my luck ran out as far as using tap water (been using tap water for years without any problems).... I have suffered massive losses in the last 6 weeks and this is the only thing I can attribute it to. Did a water change in one tank, lost 3 of 4 fish, did a water change in different tank, lost a clam and before that an angler, did a water change in my other tank and lost my cleaner shrimp, and did a water change in last tank and lost longnose hawkfish and before that swallowtail hawkfish. I have not done any water changes since the most recent loss because I was waiting for my RO/DI unit and do not want to lose anything else (holding my breath with each required top off using tap water, waiting for something else to die.... while waiting for my RO/DI unit). That list of losses about made me want to quit the hobby.

My RO/DI unit showed up a few days ago, now I need to do some massive water changes that are overdue.....
 
I lost my male potter's angel in QT a couple of weeks ago. He was in his second week of cupramine treatment, stopped eating one day and died the next. The smaller female that was in the same QT tank with him finishes her fourth week of cupramine treatment today. Cu levels were kept closer to 0.35ppm than 0.50ppm and that level was ramped up over the course of 4 days.

My most painful loss was finding my 4.5" achilles tang stuck to the intake of a modded maxijet. He was still alive, but really screwed up...so sad.
 
Saltwater wise, I lost my cleaner wrasse about 3 weeks ago due to the shield falling off my koralia (poor little guy was all sliced up).

As for freshwater, I lost a black neon tetra today. He was the largest of a school of 7 I bought roughly 4 years ago. Probably was just old, seeing as I got him already fully grown.
 
scribbled angelfish... had her for a little over 5 months..i was sitting in my chair watching the tank...she was perfectly normal acting..then she shook went stiff as a board with all fins flared and dropped dead ..she was dead before she dropped to the bottom of the tank and it all took less than 3 or 4 seconds from the start of the shake to hitting the bottom of the tank
 
that I know of is my mandarin it was about a year old but I could of lost something else I have fish go missing all the time sometimes they show up sometimes they don't
 
Blue/Green Chromis. Smallest in a group of four. Somehow made his way down into my sump tank and got caught in the skimmer pump. Poor guy got ripped to shreds. :(
 
I lost my spotted bristletooth tang over the weekend. Don't know what happened but he's nowhere in or around the tank. :(

I lost my helfrichi firefish a couple weeks ago, was in my bc29. I think the banghaii bullied him to death. Banghaii has a new home now....
