Sorry about your loss. I am planning my 180 upgrade around this fish. I do not have one yet and will not until it is up and cycled. Before I transfer anything over from my 100gal I was thinking of using the 180 (once ready) as the qt for this fish and hypo the tank with only sand, base rock and some live rock for seeding from my current tank. Any advice on this plan guys?

Not a good idea, the hypo at 1.009 (which you need to kill ich) will also kill the live rock causing ammonia etc. Unless you plan on doing huge daily water changes... which would stress the Achilles out. But thats, just my opinion

I'm sorry to hear about your loss. If you don't mind me asking, what other tangs did you have in there with him? what do you have for flow? what skimmer? how often were you feeding the achilles? I just want to see if there are some more obvious reasons why this happened.

Have done it in the past. It does not kill bacteria in LR. it does work. It will kill other things though and could cause a spike in Ammonia. I did it with plenty of water changes.
Hypo can cause other problems with the swim bladder on a Tang.

I have a small hippo tang, a regal angelfish, and an occelaris clown. For flow, I have 2 vortech mp40w that produce a crazy amount of flow in my aquarium. Skimmer is a Euro-reef RC-180. This skimmer works great and is rated for a much larger aquarium. I feed my tank 3 times a day using a combination of mysis, marine cuisine, clam on half shell, formula 2 pellets. Also, Nori is always available on a clip.

I never said it kills the bacteria. I have done hypo successfully in a QT with a bio-wheel filter which had an established bacteria population. Still needed to do lots of pwc. If you do it with live rock it will even be worse, in regards to ammonia spiking.
I finally bit the bullet. Been looking/researching achilles tangs for over a year now for my 180. When I got the tank last year I pretty much set up the entire thing with the achilles in mind. Back in June I asked my LFS owner to order me a good one in since I always seem to miss out on the DD ones. Almost 5 months later he finally got me one. Haven't seen it yet, though, he's had it at the store for a week - I've been dying to go see him but my nutty work schedule has prevented me from doing so. I'm hoping that by Thursday at the very latest he'll be all cozied up in my tank. The owner says he's absolutely gorgeous at 5 inches and apparently there's a line of people trying to snatch him out from under me. I'm like a kid on Christmas morning! :spin3: Can't wait to finally see him! Wish me luck!!
