Ugh....not sure why it decided to do this but it did. Question is will it survive!!!? It appears to have some feet left but looks like a donut. (Hole in center) didn't smell bad when I found it. I put it in a small container for few minutes while I investigated it. When I went to put it back in tank it looked like it was stuck to container a little bit. It is back in tank and as you can see from picture not the happiest. It's not falling apart or anything like that. Will it survive? Should I just leave it be for a few days? Thanks!

Yeah so far no mush. Only been 3 hours or so that it happen. I'll keep watching it through the day. Leaving powerheads off all day so it can maybe get a footing.
It needs some flow in order to not stagnate and allow debris or excrement to dwell on the nem.
One of mine was cut in half, looked worse, a couple months ago. Now I have two, one a little bigger than a quarter and the other about a nickel size.
Yeah I left them off for about hour so it could try and attach itself again. Then turned just one PH on. Seems to be doing much better now. Simply amazing they can survive that

Sometimes it even results in 2 survivors!
Attaching is a good sign, good luck, post how it goes.
Settled into a spot yesterday and stayed there all night. Looks little better this morning. Put filter sock over PH's just in case!
